The Internet of Things (IoT) is widely appreciated in many applications, including medical monitoring and healthcare, due to its low cost and autonomous operations. Recently, various wearable devices have been widely used and gained popularity in health monitoring systems.  

The monitoring system uses biomedical sensor nodes (BSNs) to detect the patient’s physiological parameters, which are sent to the coordinator or base station (BS) for further analysis via the Internet of Medical Things (IoMT) framework. However, the BSN has limited energy, and most of the energy is consumed by data transmission. Therefore, the main problem of research is to reduce energy consumption and communication costs.  

More importantly, transmitting patient data over an insecure channel can lead to security and privacy risks. Therefore, ensuring privacy and data integrity is another challenge.  

This paper recommends an efficient and secure health monitoring system to reduce the energy consumption and communication cost of the monitoring system. However, it also provides secure data transmission by incorporating a lightweight security mechanism based on Cipher Block Chaining (CBC).  

Comprehensive simulation results show that the proposed scheme achieves better performance in terms of network lifetime, end-to-end delay, and network throughput. The CBC algorithm is a suitable solution to ensure the safety of IoMT networks and extend their lifetime.  

In the intelligent exchange of health information, blockchain technology addresses existing concerns regarding security, privacy, and electronic health records. Additionally, using edge devices with IoMT devices is highly beneficial to improve security, computing, and storage challenges.  

Symmetric and asymmetric keys are used to hide sensitive information from unauthorized persons. Additionally, the SHA256 hash algorithm function helps detect data changes or integrity.  

The proposed system uses a blockchain-based smart healthcare system that uses IoMT devices for continuous patient monitoring. In addition, these various encryption keys keep data confidential within the blockchain, allowing patients to share data securely through smart contracts while preventing unauthorized doctors from viewing the data. Validation nodes and blockchains use asymmetric keys to sign and verify patient data within a healthcare provider’s system and geolocation to ensure the authenticity and source of data.
