ABCP Recertification Templates – Updated for 2003

Dear Colleagues,

Every effort has been made to reproduce the pages containing the Clinical Activity
Report (CAR) and Professional Activity Reports (PAR) recertification pages from
the American Board of Cardiovascular Perfusion accurately.

There is NO fee to down load and use these files. YOU must be sure the
contents are accurate. We assume NO RESPONSIBILITY for errors, mistakes, or
anything else that may affect the outcome of using these forms. These forms
and files are a template to help you fulfill the requirements of the ABCP CAR
and PAR recertification.

The files were made using MS WORD 2002. They are all archived into a single
“zipped” file. Download the file from the link below. Then after the
file has downloaded, use Winzip to open the file, ABCP Recertification Templates-2003.zip.
Then follow the instructions outlined below. These instructions are also included
with the download file.

The ABCP Template files have been updated for 2003. Save this file to your
computer > ABCP

Instructions:

1. Extract all of the files within the ABCP Recertification Templates-2003.zip
archive. Save the ABCP Recertification Templates-2003.zip file as a backup.

2. The files are made in a “form” format. Tab to advance to the next
form field. Box fields can be toggled with an “X” by left clicking
on the box or pressing the space bar.

3. Printing: Each file is one side of a page (or a page by itself). Pages that
require “front and back” require the printing of one side and then
reinserting the page in the printer and printing the other side (of a different
file). Follow the format sent to you from the ABCP.

I hope this helps some of you. I spent many hours on these forms because I
too could not use the WordPerfect format supplied by the ABCP. And at this point
in time, the online filing process seems too cumbersome.
